Making a change

Changing disability support providers

You have choice and control over your providers. A planned transition helps protect continuity and important information.

Review your current agreement

Check notice requirements, cancellation terms, outstanding invoices and how records or equipment will be handled.

Confirm the new provider

Discuss capacity, worker matching, proposed start dates and assessment requirements before ending essential existing support.

Share only what is needed

Decide what information should be transferred and provide consent. Sensitive information should not be shared simply because someone asks for it.

Plan the changeover

Agree the final date with the outgoing provider and commencement arrangements with the new provider to reduce gaps.
